Who is eligible for Paratransit?
How to apply for admission

To be admitted to Paratransit service, you must submit a request for admission and include the required documents. You can download a copy of the Paratransit Admission Request Form directly from the STM’s Website or obtain a copy by contacting Customer Service at 514 280-8211, Monday to Friday from 8:30 a.m. to 4:30 p.m. Our staff will answer your questions and provide you with all the information you need to fill out the form and submit your request.

Instructions:

Fill out and sign Part 1 "General Information" of the form. Attach a passport-size photo (approximately 3 cm x 3,5 cm) and a photocopy of your health insurance card or birth certificate;
Have an authorized professional (optometrist, psychologist, doctor) fill out and sign the appropriate "Diagnosis" sheet(s) in Part 2;

If the person applying for Paratransit service has more than one disability, the corresponding diagnosis sheet must be filled out for each impairment:

Visual impairment,
Psychiatric disability,
Intellectual disability,
Motor or organic impairment.

The Admissions Committee will inform you of its decision in writing within 45 days after receiving the completed form and attached documents. Please note that any application found to be incomplete or illegible will be returned to applicant, which could delay the admissions process, as well as access to paratransit service. It is important that all questions relating to your situation be answered and all necessary documents (photo and proof of birth) be included. Be sure to sign the form as required on page 3.
