Aide Transit Pass

| Please note that customers of the Paratransit service do not have to fill out this application, their transit pass being accepted as a Personal Aide Transit Pass on the regular network. |
How to apply?
To receive the Personal Aide Transit Pass, fill out part 1 and part 2 of the appropriate application form, join a photograph of yourself and forward it to any of the rehabilitation centres identified on the form.
At the chosen Centre, an health professional will complete part 3 of the application and forward it to the STM. A Personal Aide Transit Pass will then be issued and sent to you.
How much does it cost?
The Personal Aide Transit Pass is free of charge. Through this program, the STM hopes to facilitate the use of its regular transit system for people with a mobility impairment.
What are the conditions of use?
As a holder of a Personal Aide Transit Pass, you agree:
- to show the pass when boarding an STM vehicle or entering a metro station;
- to inform the STM of any change of address;
- to inform immediately the STM if the pass is destroyed, stolen or lost;
- not to let anyone else use the Pass;
- to return immediately the Pass to the STM if unused.

personnes handicapées du Québec (OPHQ), the R�gie r�gionale de la sant� et des
services sociaux de Montréal-Centre and Montreal's rehabilitation centers for
intellectually or a visually impaired people, make it possible to obtain a free
Personal Aide Transit Pass.
